Supports Program
Specifically in the Self-Directed Supports (SDS) program,
Dane
County Human Services determines whether an
individual is eligible to be included in the program
and the County will then determine an individual rate
of financial support that the County would commit to
for an individual. That amount is communicated to the
individual and their community support broker. (The
term community support broker refers to a person hired
to advocate and help make decisions for the individual
with disabilities.) The client and the broker select
and hire individuals or agencies to provide services
and assistance that the client needs and/or wants in
their lives. They submit a budget to the Dane County
Human Services staff requesting approval of the purchased
services. The client and their broker submit signed
authorizations to the fiscal administration program
(Fiscal Assistance of Dane County, Inc.) requesting
that the agencies or people be paid. The County sends
1/12th of the total yearly rate each month for that
individual to Fiscal Assistance. Fiscal Assistance holds
those funds in the client's name and pays the appropriate
persons/agencies based on the signed authorizations
received from the client and broker. Each month Fiscal
Assistance provides statements to the client, broker
and guardian, if applicable; detailing what was actually
received and spent as compared to the approved budget.

Rep
Payee Program
What happens when Fiscal Assistance of Dane County acts
as Rep Payee? Social Security monthly benefits are directly
deposited into an account for F.A.'s rep payee clients.
Monthly bills are then mailed directly to Fiscal Assistance.
F.A. sends payments for rep payee bills out weekly.
The consumer has peace of mind, knowing that the bills
are paid on time. A regular spending allowance is sent
out based on the consumer's needs and budget. A statement
is sent out at the end of each month to the client,
showing the amount of the monthly benefit received,
which bills were paid, how much was paid, and what the
current account balance is. FA completes all reports
required by the
Social Security Administration to account
for how the funds were spent.
